Work Related Injuries

·        In case of a life threatening emergency, an employee should report DIRECTLY to the nearest hospital emergency room.

Employee Responsibility

• Employee should notify supervisor of injury as soon as possible
  • Supervisor will complete LHUP Work-Related Injury Report available at: http://www.lhup.edu/HR/Worker's_Compensation_Directory.htm

• Any employee injured on the job, but not needing medical care should contact
  Kim Powell, 570-484-2486
   
• Kim will need a completed LHUP Work-Related Injury Report from the supervisor

·        Any employee injured on the job and requiring medical care will:

·        Contact Kim Powell, 570-484-2486

§         Kim will need a completed LHUP Work-Related Injury Report from the supervisor.

§         Kim will complete the First Report of Injury with Inservco, Inc.

·         The employee will seek medical care for the first 90 days of an injury from ANY panel physician.  Panel listing is available at:  http://www.lhup.edu/HR/Worker's_Compensation_Directory.htm

Supervisor Responsibility

o       Complete an LHUP Work-Related Injury Report

·        Report can be located at: http://www.lhup.edu/HR/Worker's_Compensation_Directory.htm

o       Forward completed Investigation Report to Kim Powell. J205 East Campus

o       Maintain contact with injured employee regarding return to work.
