Disability Related Accommodation Procedures


REQUEST FOR ACCOMMODATIONS                           Accommodation Form

Lock Haven University has established the following procedure for individuals to request information and/or assistance in relation to the Americans with Disabilities Act of 1990. 

If the person receiving the request is not able to provide a solution to the request, he/she should provide the requester with a copy of the grievance procedures.  

A.  Any individual needing information and/or assistance, as defined by the ADA, Section 504, should contact one of the following Lock Haven University employees:
