BI-WEEKLY EMPLOYEE 10% CONTRIBUTIONS

Effective January 1, 2005, employees will be required to contribute to the cost of their health care.  Employees will contribute 10 percent of the premium for their health and prescription drug coverage.  Costs will be calculated on the type/size of contracts.  For employees enrolled in the Indemnity or PPO Plan, rates will be based on the following categories: single, two-party, or family.  HMO rates will be based on either a single or family size contract.   All contributions will be made through bi-weekly pre-tax payroll deductions.

 

Below are employee costs for participation in the health and prescription drug plan in the Pennsylvania State System of Higher Education Group Health Program.  These contribution rates are effective January 1, 2005.
