Please follow these steps to obtain services based on your needs:
Accommodation Letters for Professors: Please see Jean Mason,
Secretary for Disability
Remember that accommodation
letters need to be provided to Books on Tape: Please obtain a Books on Tape Request Form from http://www.lhup.edu/jmason1/, complete, and e-mail it to Jean Mason at jmason1@lhup.edu. Please note that if we are able to obtain books on tape from our provider, they usually take two at at least (2) weeks to arrive. If we cannot obtain the books from our provider, Jean Mason will ask you to provide a course syllabus so that we can have someone read the chapters that you need. Live one on one readings are also avialable. Note Takers: Please obtain a Note Taker Request Form from http://www.lhup.edu/jmason1/, fill out the form and email it to Jean Mason at jmason1@lhup.edu. Once notes are available, they will be placed outside of the box in front of Jean Mason’s office in Room 23 Russell Hall (see directions to Room 23 under “Accommodation Letters”). Please be aware that note taking services will be cancelled if: 1) You do not or rarely attend the course or2) You do not pick up your notes at least twice a week. Extended Time for Exams in Disability Service’s Quiet Room: Please contact Jean Mason, Secretary for Disability Services at jmason1@lhup.edu or (570) 484-2665 to meet with her to schedule your exams. If a professor gives an unscheduled exam, we require that you notify Jean Mason at least five (5) working days notice in order to make the necessary accommodations. Scheduling an Appointment with a Counselor: Please contact Jean Mason, Secretary for Disability Services at jmason1@lhup.edu or (570) 484-2665. Please provide Jean Mason with all of the times that you are open and she will schedule you into the next available appointment time.
