Electronic Library Forms

Request an Interlibrary Loan book or article - Interlibrary loan is for use by LHU students, faculty, and staff. Community patrons are required to use the interlibrary loan service provided by your local public library.

Microform Copies Delivered to your Desktop - LHU patrons may request copies of articles from microfilm or microfiche free of charge. Documents will be scanned in PDF files and delivered to the requestor via an LHUP email address. This request is completed through the ILLIAD system. Please click the Request a Photocopy button in ILLIAD.

New Book Requests - Faculty and students are encouraged to submit recommendations for new books to the Lock Haven University Libraries. To submit a book you would like to recommend for purchase, please fill out our request form.

Reference Questions - If you are unable to visit Stevenson Library's Reference Desk during regularly scheduled hours, check here for other methods to contact us for information.

Reserve Request Form for Books - This printable form can be used by faculty members to put books on reserve in the library.

Reserve Request Form for Photocopies - This printable form can be used by faculty members to put photocopies on reserve in the library.

Reserve Request Form for Media - This printable form can be used by faculty members to put media on reserve in the library.

Reserve Request Form for Eres - This printable form can be used by faculty members to put documents on Electronic Reserve in the library.

Stevenson Library Laptop Agreement - This printable form can be printed out by students who would like to check-out a laptop from the Circulation Desk. This form must be completed and signed before checking-out a laptop.
