Terms and Conditions
All individuals requesting an LHU OneCard must complete and sign an LHU Cardholder Agreement form. Completion of this form constitutes an acknowledgement of the cardholders’ rights and responsibilities with respect to their official university identification card. The LHU OneCard is the official identification card for all LHU students, faculty, and staff members. The LHU OneCard is required for all active members of the LHU community.
Regulations

Rights and privileges associated with the card are non-transferable.

Rights and privileges associated with the card are contingent on active status as a student, faculty, or staff member.

The card must be carried at all times and presented to an LHU official upon request.

Photo images captured for the identification card become part of the cardholder’s educational or employment record and therefore may be used for official university business.

The cardholder will have the responsibility for reporting and replacing lost, stolen, or damaged cards on the next business day.

The cardholder is responsible for maintaining the security of all PIN numbers associated with services related to the LHU OneCard.

The cardholder is issued the first card at no charge. Subsequent cards issued to the same cardholder will be considered replacement cards and are subject to a $15 replacement fee.

Cardholders will not be allowed to wear any article of clothing or eyewear that, in the judgment of the carding staff, will obscure their physical features when their photograph is taken for the LHU OneCard. Any exceptions to the above will be handled on a case by case basis.

The LHU OneCard can be used for University Dining Services Meal Plans and Flex Dollars, and for ATM transactions through campus banking services. Because of this, the LHU OneCard should be treated as cash and kept in a secure place at all times. Cardholders shall not loan their cards to anyone at any time.

Card Issuance

The LHU OneCard will be issued to you when you first enroll as a student at Lock Haven University or upon employment. It is your official identification card and is valid as long as you are an enrolled student or an employee of Lock Haven University. The LHU OneCard is also issued to individuals affiliated with Lock Haven University, based on specific criteria, determination of which is at the discretion of the University.

New cardholders must provide a valid photo ID before receiving their official LHU OneCard.

Each student, employee and affiliate is entitled to carry only one active card, except in cases where the access requirements of the individual exceed the capacity of one card. Employees who are taking classes will not receive a student card. For the purposes of the LHU OneCard, student cards are issued to both undergraduate and graduate students.

The name printed on the LHU OneCard is the individual’s official name as recorded in the LHU administrative database.

No profiles, hats, headbands, sunglasses, etc. are allowed when your photograph is taken for the LHU OneCard.

At the sole discretion of the University, cards shall immediately be surrendered to University Officials upon request. Failure to do so shall result in the deactivation of the card and potential student discipline for failure to follow university rules and regulations. The cards are the property of the University and issued to students for university related or student affairs purposes and have no life span beyond the matriculation of the student.

If your LHU OneCard is replaced due to theft or loss, it is deactivated and cannot be reactivated should you find it at a later date.
Card Composition

Magnetic Stripe: The magnetic stripe on the back of your LHU OneCard can be used for meal plans/flex dollars and banking through campus banking services, if desired.

Proximity Transmitter: There is a proximity transmitter embedded in your card (you cannot see any part of it). It is used to grant access to authorized doors and to the Student Recreation Center.
Safeguards

Protect your LHU OneCard from damage by keeping it away from abrasive surfaces.

Never lend your card to others.

Do not punch holes, affix stickers (other than the residence hall sticker), or in any other way make modifications to your LHU OneCard. Such practices may create problems when trying to use your card for official university business or card-swipe functions.

Avoid bending the card or using it for any other purpose than those outlined above.

Avoid exposing the card to magnets, chemicals, detergents, dirt, and excess water.

Prevent direct magnetic-stripe-to-magnetic-stripe contact with other cards such as a driver’s license or credit cards.

Clean card with water only applied to a damp cloth. Do not submerge.
