Summary of Academic Program Assessment

The Office of Planning and Assessment requests that each academic program complete a Summary of Program Assessment form.  On this form, each program lists primary learning outcomes goals for students, when and how those goals are to be monitored, expectations for performance, observations of actual student performance, and program follow-up or changes that may be necessary.  

Dr. Douglas Eder of Southern Illinois University at Edwardsville recommended the Summary of Program Assessment form as a means of reporting how well departments were implementing learning outcomes assessment plans.  The LHUP Outcomes Assessment Committee, comprised of representatives from each department, endorsed the use of the form as an easy and concise way in which to track and report progress.  The information gathered from the summary form can assist departments in the Five-Year Program Review process, serve as a way to communicate progress, and facilitate external reporting requirements.
