You may fax or mail the completed form. Unfortunately, we can not accept forms via email, as you must submit your registration fee. The registration fee is $20.00, which includes lunch, snacks, and beverages at the conference. Please make your check payable to Lock Haven University of PA. Attendees who pre-register will receive a visitor's parking pass in the mail.
