Tryouts are for enrolled students only!

TRYOUT PROCESS:

1.  Complete and submit the form below.

2.  Complete the Athletic Insurance Affidavit form and bring to mandatory meeting  (you can find the form by clicking on the link below)

3.  Attend the mandatory meeting on Monday, August 31st at 5:30pm in the Thomas Fieldhouse classroom. 

4.  Fill out and return the Athletic Insurance Affidavit form.  You can find this form on our website at http://www.lhup.edu/deptofathletics/handbook.htm.  This form requires a parental signature. Please bring this form to the mandatory meeting.  You may not participate in tryouts without a completed Athletic Insurance Affidavit.

5.  Please be certain that Health Services has received a copy of your physical.  You may not participate in tryouts without a completed physical.  You can contact Health Services at (570) 484-2276.

6.  Attend tryouts on Thursday, Sept. 3rd and Friday, Sept. 4th (if necessary).  Time will be determined at the mandatory meeting.
