Tryouts are for enrolled students only!

 

TRYOUT PROCESS:

1.  Complete and submit the form below.

2.  Complete the Athletic Insurance Affidavit form and bring to mandatory meeting  (you can find the form by clicking on the link below)

    http://www.lhup.edu/deptofathletics/forms.htm  This form requires a parental signature. Please bring this form to the mandatory meeting.  You may not participate in tryouts without a completed Athletic Insurance Affidavit.

3.  Attend the mandatory meeting on Monday, August 30st at 5:00pm in the Thomas Fieldhouse classroom. 

4.  Please be certain that Health Services has received a copy of your physical.  You may not participate in tryouts without a completed physical.  You can contact Health Services at (570) 484-2276.

5.  Attend tryouts on Wednesday, Sept. 1st.  Time will be determined at the mandatory meeting.
