The level of the urbanicity treatment variable is described on page 2 (478) of the article. It is based on the number of addresses per squared km. There are five levels. The familial liability treatment variable had three levels and is described on page 2 as well. The levels are based on the existence of psychosis in immediate family (yourself, an immediate family member, and an immediate family member who had received treatment). Hope this helps!
