VA Education Benefit Application Process Note:

applicants
apply by completing VA Form 22-1990, the Application for VA Education
Benefits. Attach a copy
of DD Form 214 (i.e. discharge papers) to the application before
returning it to the LHUP Student Financial Services Office for processing. Transfer
applicants (i.e.
students who received the benefits at a former school) apply by
completing VA Form 22-1995, a Request for Change of Program or Place
of Training form. Completed
forms should be returned to the LHUP Student Financial Services Office for processing. No additional forms are required. Renewal applicants. To coordinate payment of benefits for the upcoming summer sessions and academic year, the LHUP Student Financial Services Office contacts benefit recipients annually (usually in March) via a mailing to their local addresses. To avoid a lapse in payment, renewal applicants are encouraged to respond to the mailing as soon as possible. All Chapter 30 applicants must verify their enrollment each month to ensure continued payment of benefits. Enrollment may be verified via either touch-tone telephone at 1-877-823-2378 (toll free) or the DVA's web site at www.gibill.va.gov - follow the link to the Web Automated Verification of Enrollment (WAVE) program. completing VA Form 22-1995, a Request for Change of Program or Place

and Dependents' Educational Assistance (DEA) New

22-5490, the Application for Survivors' and Dependents' Educational
Assistance. If the
student is an eligible son or daughter who is under age 18, a parent
or guardian must also sign the application.
Completed forms should be returned to the LHUP Student Financial Services Office for
processing. No additional
forms are required. Transfer
applicants (i.e.
students who received the benefits at a former school) apply by
completing VA Form 22-5495, a Request for Change of Program or Place
of Training. Completed
forms should be returned to the LHUP Student Financial Services Office for processing. No additional forms are required. Renewal applicants. 31 - VA's Vocational Rehabilitation and Employment (VR&E) New
applicants
apply by completing VA Form 28-1900, the Disabled Veterans Application
for Vocational Rehabilitation. The
veteran may request this form from the VA by calling 1-800-827-1000.
Completed forms should be returned to the VA. Transfer
applicants (i.e.
students who received the benefits at a former school) should contact
the VA at 1-800-827-1000 to discuss the transfer of benefits to LHUP. Renewal applicants. To coordinate payment of benefits for the upcoming summer sessions and academic year, the LHUP Student Financial Services Office contacts benefit recipients annually (usually in March) via a mailing to their local addresses.
